Characterizing Temporal Dynamics of Sleep as a Function of Heart Rate Variability: A Digital Mental Health Approach

Sleep disruptions are a well-known phenotype of mental illness, often associated with physiological stress and cardiovascular changes. Yet, the mechanisms underlying this association remain poorly understood, in part because physiological measures are typically captured in time-limited lab settings (e.g., electrocardiogram, polysomnography). Here, we leverage passive wearable technology (Oura Ring) to continuously monitor sleep and HRV in 49 participants as part of a larger pharmacological lab study. We characterize sleep across the 7 nights following a stress event (defined as a night when a participant's HRV falls at or below their personal 20th percentile) and find these events are associated with persistently reduced sleep efficiency and to a lesser extent, reduced deep sleep, providing novel insight into how physiological stress shapes temporal dynamics of sleep.
